Question Guidance
1. Please select the hospital(s) you are completing this form for The name of the hospital or hospitals (if you have a combined service) that you are completing the form for. The form has room for up to four hospitals. If your service spans more than four hospitals please list them in the “additional comments” section at the very end of the form.
2. Are you filling out this survey for the Paediatric or Adult service? The age-group your service treats. If the service covers both Paediatric and Adult patients please complete one copy of the form for each.
3. How many local consultants work in the Joint Cardiac Clinics? A snapshot now (at the time of completing the form) of the number of consultants working in Joint Cardiac Clinics. Responses can be exact (preferred) or an estimate (including an estimated range).
4. How many urgent referrals do you currently have awaiting review? A snapshot now (at the time of completing the form) of the total urgent referrals that are awaiting review. “Awaiting review” is meant as referrals that have been received but not yet put on a waiting list / given appointments / rejected etc. Responses can be exact (preferred) or an estimate (including an estimated range).
5. How many non-urgent referrals do you currently have awaiting review? See Q4 above (but for non-urgent referrals).
6. During the reporting quarter, what was the average wait (in weeks) for a new patient to receive their first appointment? The average wait – in weeks – for patients who had their first outpatient appointment during the three month period specified. Responses can be exact (preferred) or an estimate (including an estimated range). If providing an exact response, this would usually be sum total of days waited by all patients divided by the total number of outpatient appointments, then divided by seven. Please include attendances and DNA’s but not cancellations when calculating this value. If you are providing an exact response you will also be asked for the total appointments and total days waited that were used to calculate this average (providing this allows us to also calculate network-wide and regional average waits).
7. What is the total number of individual patients under PEC or Adult Cardiologist screening clinics? A snapshot now (at the time of completing the form) of the total number of unique individual patients who either have an appointment booked or are on a waiting list for an outpatient appointment in a PEC of Adult Cardiologist screening clinic. Responses can be exact (preferred), an estimate (including an estimated range) or N/A if you do not have PEC / Adult Cardiologist screening clinics. This question is for Level 2 & 3 centres. Please choose N/A if you are a Level 1 centre.
8. What is the total number of individual patients under Joint Cardiac Clinics? See Q7 above (but for Joint Cardiac Clinics). This question is for Level 2 & 3 centres. Please choose N/A if you are a Level 1 centre.
9. What is the number of overdue follow-ups for the PEC or Adult Cardiologist screening clinics by 12+ months? A snapshot now (at the time of completing the form) of the total number of unique individual patients who are waiting for follow up in a PEC or Adult Cardiology screening clinic who were due for follow up appointments 12 or more months ago at the time of completing the form. Please include both patients on a waiting list and those with booked appointment dates that are 12 or more months later than the original date intended. Responses can be exact (preferred), an estimate (including an estimated range) or N/A if you do not have PEC / Adult Cardiologist screening clinics. This question is for Level 2 & 3 centres. Please choose N/A if you are a Level 1 centre.

11. During the reporting quarter, what was the average DNA rate (%) for the PEC or Adult Cardiologist screening clinics? The DNA rate – as a percentage – for non-cancelled PEC or Adult Cardiologist screening clinic appointments during the three month period specified. Responses can be exact (preferred), an estimate (including an estimated range) or N/A if you do not have PEC / Adult Cardiologist screening clinics. If providing an exact response, this would usually be the total number of DNA’s divided by the total number of appointments (attendances + DNA’s), then multiplied by 100. Please include attendances and DNA’s but not cancellations when calculating this value. If you are providing an exact response you will also be asked for the total attendances and DNA’s that were used to calculate this average (providing this allows us to also calculate network-wide and regional DNA rates).This question is for Level 2 & 3 centres. 13. During the reporting quarter, how many Paediatric Cardiology or ACHD inpatient inter-hospital transfers to another Lifelong CHD Network Centre occurred? The number of inter-hospital transfers for Paediatric Cardiology or ACHD patients out of your provider(s) to another centre within the Lifelong CHD Network* during the three month period specified. Responses can be exact (preferred) or an estimate (including an estimated range).
14. During the reporting quarter, what was the average wait for inter-hospital transfer to another Lifelong CHD Network Centre, in days? The average wait – in days – for an inter-hospital transfer for Paediatric Cardiology or ACHD patients out of your provider(s) to another centre within the Lifelong CHD Network*during the three month period specified. Days waited are the difference in days between the date a patient would ideally have transferred and the date the actual transfer occurred. Responses can be exact (preferred) or an estimate (including an estimated range). If providing an exact response, this would usually be the sum total days waited for transfer divided by the number of transfers (the total provided in Q13). If you are providing an exact response you will also be asked for the total days waited that was used to calculate this average (providing this allows us to also calculate average network-wide and regional waiting times).
15. During the reporting quarter, what was the longest wait a patient experienced for inter-hospital transfer to another Lifelong CHD Network Centre, in days? The longest single wait – in days – for an inter-hospital transfer (as defined for Q14) for a Paediatric Cardiology or ACHD patient out of your provider(s) to another centre within the Lifelong CHD Network*during the three month period specified. Responses can be exact (preferred) or an estimate (including an estimated range).
