We are seeking expressions of interest on our Network Board for the position of a Paediatric/Fetal Patient & Public Voice (PPV) Representative.

If you are a CHD patient, family member or carer with a keen interest in improving patient care we invite you to complete an expression of interest via email to chdnetwork@gstt.nhs.uk by Friday 31st May. This will require you to answer the following questions (100 words max per question): 

1. How would you describe yourself (how do you work with others, how do you make your opinion count)? 
2. Why are you interested in the Lifelong CHD Network? 
3. How can you make a difference and what experience will you bring?
